I am at least 19 years old

I will eat a big breakfast/lunch prior to my tattoo and will try to be well rested. If I feel hungry, lightheaded or "off" at all during my tattoo appointment I will notify Shy Alexx immediately so I can receive the necessary care

I understand that Shy Alexx will NOT copy any design, reference photos will be used as inspiration ONLY

I understand that I will not receive a copy of my design prior to the tattoo appointment as it is Intellectual Property

I am not pregnant, nursing, on antibiotics, or have a skin condition (if you cannot check off this box, you must disclose your circumstance prior to booking in)

I understand that a NON-REFUNDABLE deposit will be required prior to confirming my booking, this deposit will be discounted off of the LAST hour of my tattoo

There is absolutely zero tolerance for rude or offensive behaviour, Shy Alexx has the right to refuse my service should I treat her with disrespect

I understand that tattoos in high flexion and friction placements (neck, knees, feet, hands) are NOT guaranteed due to the skin composition. Should I choose this placement and require a touchup, it will be charged for accordingly.

I understand that if I book in with Shy Alexx at an Incorporated Studio I am responsible for reviewing that Studio's policies prior to sending a deposit. If I send a deposit to book my appointment, I am agreeing to all of the policies of that Studio.
